A etiologia da doença de Kawasaki é desconhecida, mas a epidemiologia e a apresentação clínica sugerem infecção ou resposta imunológicia anormal a uma infecção em crianças geneticamente predispostas. Doença autoimune também é uma possibilidade. Embora as crianças descendentes de japoneses tenham uma alta incidência, a DK ocorre no mundo todo. Nos EUA, registram-se 3.000 a 5.000 casos/ano. A proporção homem:mulher é cerca de 1,5:1. Oitenta por cento dos pacientes têm < 5 anos de idade (pico, 18 a 24 meses). São raros os casos entre adolescentes, adultos e < 4 meses de idade.

Os casos ocorrem o ano todo, mas são mais frequentes na primavera ou inverno. Grupos com DK têm sido relatados em comunidades, sem evidências claras de uma disseminação de pessoa para pessoa. Cerca de 2% dos pacientes apresentam recorrências, geralmente meses a anos mais tarde. Não há prevenção conhecida.
